Right to Request a Form 1095-C

Your Form 1095-C will be ready by February 28, 2026.  In accordance with federal law, Destin Fire Control District does not mail the Form 1095-C.  Employees and /or former employees still have the right to request their Form 1095-C once the form is available.  Contact (850) 837-8413 to request a copy, send request to contact@destinfire.com, or mail to 848 Airport Road, Destin, FL  32541.

The Employer Reporting Improvement Act and the Paperwork Burden Reduction Act (the Acts) state that employers are not required to send the Form 1095-C to all full-time employees and covered individuals. As an employee, you have a right to request a Form 1095-C. This means paper forms will no longer be mailed.
